怎么做课程描述的statutory declaration?

以前看过一个帖子,说是评估CPAA的课程概要可以不拿回学校盖章,只是自己写个声明,就是这段了:You must prepare your own summary of topics covered and submit it with a statutory declaration that the information given in the summary is true.
记不清到底是怎么做的了,有谁知道吗?
