请教大家申请medicare 的declaration of residency

我第二次landing,去申请medicare, 工作人员说我属returning resident, 需要提交declaration of residency, 那张表的第二项要求make the following declaration under the Statutory Declarations Act 1959:这部分应该填写什么啊？有谁办过，请指教指教！
另外，见证人怎样找啊？
