Lost opportunity to save Elu's life: coroner

A Sydney hospital's failure to test an eight-year-old boy for meningitis was a "lost opportunity" to save his life, a coroner has found.
Isaraelu Pele, or Elu, died from bacterial meningitis at Bankstown Hospital on December 18, 2007.
The inquest was told he went to Bankstown Hospital on December 16 and the Children's Hospital at Westmead the next day but was discharged despite suffering from severe vomiting, headaches and lethargy.
He had also seen his family doctor twice and another GP since becoming ill on December 14.
The inquest was told that a lumbar puncture, thought to be the definitive test for meningitis, was not carried out during his first visit to Bankstown Hospital.
Deputy State Coroner Hugh Dillon today said that doctors considered the diagnosis of meningitis but did not conduct the tests.
"This was a lost opportunity to save Elu's life," Mr Dillon told the Coroner's Court in Glebe.
"Instead, the diagnosis reached was that he was suffering from a relatively benign viral gastroenteritis."
Mr Dillon's formal finding was that Elu died of bacterial meningitis that had not been diagnosed by a number of clinicians who examined him.
However, he acknowledged the difficulty for doctors at both hospitals and for the GPs, in that Elu did not appear to have shown the classic signs of the condition, including neck stiffness, a rash and photophobia.
Mr Dillon made 11 recommendations, including that the Sydney South West Area Health Service and the Children's Hospital provide for senior staff to assess children presenting with signs of toxicity before they are discharged.
Elu's parents and two of his siblings supported each other as the coroner read out his findings.
