How To Apply for Seniors Card
A Seniors Card Application Form must be completed when applying for the Seniors Card. To be eligible, you must be a New South Wales permanent resident, aged 60 or over and work no more than 20 hours a week in paid employment.
The form must be witnessed by a Justice of the Peace to verify that you meet membership criteria.
Please note that as of 8 December 2006, it became a statutory requirement that all existing JPs be reappointed and assigned a new registration number. This new registration number must be included on all new member application forms. Visit the NSW Lawlink website for further information on this reappointment process.
You can obtain a NSW Seniors Card application form from one of the following locations:
